Documents you need for Fair Deal
The single biggest reason Fair Deal applications stall is missing supporting documents. The HSE will not progress without them — they evidence what's written on the form. This is the practical checklist, organised by what triggers each document.
Always required (everyone)
- Photo ID — passport or driving licence for the applicant.
- PPS number proof — public services card, MyGovID printout, or tax/Revenue document showing the number.
- Birth certificate — long-form preferred.
- Most recent 6–12 months of bank statements for every account (current, deposit, online, credit union, post office, An Post).
- Most recent pension statements (State pension, occupational, foreign).
- Last 2 years of P21 / Statement of Liability from Revenue if you have employment, self-employment, or rental income.
If you own a home (Part 3C)
- Certificate of Market Value from a registered auctioneer or valuer — dated within the last 6 months. Self-assessed estimates are not accepted.
- Mortgage statement if there is a loan on the property — current monthly repayment and outstanding balance.
- Folio number if registered with the Land Registry — useful for the loan if you take it.
- Local Property Tax (LPT) receipt — for the levies-required-by-law deduction.
If you have rental income
- RTB (Residential Tenancies Board) reference number for each tenancy — required where applicable.
- Most recent year's rental income & expenses — typically the rental section of the Form 11 / 12.
If you have ARFs / AMRFs / shares / bonds
- Most recent statement from the QFM (Qualifying Fund Manager) for each ARF / AMRF.
- Stockbroker statement showing share holdings and most recent valuation.
- Bond / security certificates or holding statements.
- Life policy encashment value letter or statement from the insurer — only policies with a surrender value are assessable.
If you've transferred assets in the last 5 years
- Documentary evidence of the transfer — sale contract, gift letter, deed of transfer.
- Valuation at the time of transfer from a registered auctioneer or valuer.
- Evidence of any returned assets — bank statement showing the inflow if any of the transfer was reversed.
If you're applying for the Nursing Home Loan (Part 5)
- Folio number for each property to be charged.
- Land registry compliant map — required if the principal residence is contained within a folio that includes other land. Must be original or colour copy, certified by an Engineer / Architect, and outline the secured portion in red.
- Relevant Accountable Person details (name, address, phone, eircode, PPSN) — they will be the HSE's point of contact for the loan.
- Joint owners' consent (Part 5B) — if anyone else jointly owns the property, they must each sign.
If you're applying for the 3-year cap on a farm or business (Part 6)
- Proof the farm has been actively worked — at least one of: Bord Bia QAS application, valid herd-keeper number with status, registered farm partnership number, sheep/poultry/EPRN/pig/equine premises numbers, BISS / BPS application, or a copy of the herd test from the last year.
- Proof the business has been actively worked — at least one of: Revenue registration, business registration with the HSE, auctioneer's licence, publican's licence, food business registration, registration of legal or professional body relevant to the business, or proof of a Statutory Declaration on the business.
- Family successor's PPSN, DOB, address, contact details.
- Joint owners' consent (Part 6C) — if anyone else jointly owns the farm or business asset.
If you have a co-decision-making agreement (Part 7)
- Decision Support Service (DSS) registration confirmation for each co-decision-making agreement.
- Co-decision-maker's contact details — address, phone, mobile, email.
For couples (Part 1 / Part 4B)
- Marriage certificate or civil partnership cert.
- Spouse's PPSN, DOB, and most recent pension statements.
- The same income, asset, and deduction documents listed above for the spouse — the HSE assesses both halves.
If a specified person is applying on behalf of someone
Where the applicant lacks capacity to apply themselves, the application is signed by a "specified person" (Part 4B). They must provide:
- Documentary evidence of their authority — Enduring Power of Attorney instrument, Decision-Making Representative court order, registered social-worker authority, etc.
- Their own contact details and PPSN.
