What is the Fair Deal application form?
The Fair Deal application form is the official document you complete to apply for the Nursing Homes Support Scheme (NHSS) — the HSE scheme that helps with the cost of long-term nursing home care in Ireland. The same form covers the care needs assessment and a full financial assessment of your income and assets, and it decides how much you contribute each week towards your care.
The form is free from your Local Nursing Homes Support Office or from hse.ie. It runs to about 40 pages, and for most families the hard part isn’t getting the form — it’s working out which sections apply, what each question means, and which documents to gather.
The 7 parts of the form
The HSE application form is organised into seven parts. Parts 1–4 apply to everyone; Parts 5–7 are optional and only apply if they’re relevant to you.

Personal details, PPS number, marital status, address and GP. Also your spouse or partner’s details if you have one.

Care needs assessment
The declaration for the care needs assessment — signed by you, or by a “specified person” acting on your behalf.

Income, assets and deductions
Pensions, other income, savings, ARFs, your home and any assets transferred in the last 5 years — for you and your spouse/partner.

Application for State Support
The main declaration applying for financial support towards your nursing home costs.

Nursing Home Loan (optional)
Ancillary State Support — the optional loan that lets you defer the contribution based on your home until later. Only if you opt in.

3-year cap on a farm or business (optional)
For family farms and relevant businesses, where a family successor continues to run it. Only if it applies to you.

Co-decision-making agreement (optional)
Details of a co-decision-maker registered with the Decision Support Service, if you have one.
What you need to have to hand
The financial parts of the form ask for supporting evidence. It helps to gather these before you start:
- Recent bank and savings account statements
- Pension statements and proof of any other income
- A recent valuation of your home and any other property
- Details of assets (shares, ARFs, land) and any transferred in the last 5 years
- Your PPS number and photo ID
- Details of any loans or borrowings secured on your assets
